If your student has COVID symptoms or tests positive for COVID-19 ON or AFTER Jan. 15, please complete this online form. The information collected will enable BISD to determine your child’s return to school date. NOTE: Only submit this form if your student’s positive case has NOT been previously reported to the campus/nurse.
